METAL/NONMETAL MINE FATALITY - On May 19, 1995, a 5 year old boy and a crusher operator were killed at a sand and gravel operation. The boy was brought to the mine by his mother who is employed at the operation. The boy was in a school bus that was being used as an office and the crusher operator was standing outside the crusher when an estimated one million cubic yards of material collapsed from the highwall engulfing them. The victims died from crushing injuries. These deaths make a total of 18 fatalities reported in calendar year 1995 in the metal and nonmetal mining industries. As of this date in 1994, there were 15 fatalities reported in these industries. These deaths make a total of three fatalities classified as fall of face/highwall this year. There was one fall of face/highwall fatality during the same period in 1994.
