METAL/NONMETAL MINE FATALITY - On July 19, 1995, a delivery truck driver was killed at a molybdenum mine. The truck driver was helping the forklift operator unload h-beams from the delivery truck. He was standing on the beams giving hand signals to the forklift operator. The forklift operator tilted a load of beams onto the forks and noticed the truck driver on the ground. At that instant, the forklift operator heard an h-beam fall from the forklift load. The beam hit the truck driver, and he died from sustained injuries. This is the 22nd fatality reported in calendar year 1995 in the metal and nonmetal mining industries. As of this date in 1994, there were 26 fatalities reported in these industries. This death is the ninth fatality classified as powered haulage this year. There were 12 powered haulage fatalities during the same period in 1994.
