METAL/NONMETAL MINE FATALITY - On February 9, 1996, a supervisor with 10 years of mining experience was killed at a limestone plant. Four contractor employees, including the supervisor, were removing a 9-foot long section of castable liner from the 12-foot diameter rotary kiln. They had removed a key strip along the bottom of the liner and were in the process of chipping out the sides when the top section of the liner collapsed. One employee died at the scene, and the other three were injured. The supervisor died on February 14, during an operation on his injuries from the accident. This is the sixth fatality reported in calendar year 1996 in the metal and nonmetal mining industries. As of February 14, 1995, there were seven fatalities reported in these industries. This death is the second fatality classified as handling material this year. There were no handling material fatalities during the same period in 1995.
