METAL/NONMETAL MINE FATALITY - On May 18, 1996, a supervisor with 50 years of mining experience was killed at a gold mine. The employee had backed a 2 � ton dump truck down an access road to an area of the roadway that was being widened. His right rear dual wheels traveled over the roadway edge, and before attempting to pull back on the roadway, the victim decided to dump part of his load. As the victim raised the bed, he exceeded the truck's center of gravity causing the truck to tip over and roll 1 � times down the embankment. The truck did not have doors or seat belts. The victim died of injuries he received when he fell, or jumped from the truck, striking his head on a rock just below the roadway. This is the 22nd fatality reported in calendar year 1996 in the metal and nonmetal mining industries. As of this date in 1995, there were 16 fatalities reported in these industries. This death is the eighth fatality classified as powered haulage this year. There were seven powered haulage fatalities during the same period in 1995.
