METAL/NONMETAL MINE FATALITY- On July 9, 1999, a 29-year-old contractor employee was fatally injured when the grader he was driving went over a sloped inclined service road and crushed him.

BEST PRACTICES
  • Mobile equipment operators must exercise the highest degree of care and attention at all times.

  • Berms or guardrails should be maintained on the banks of roadways where a drop-off exists.

  • Roadways should be designed to reduce steep grades.
  • This is the 24th fatality reported in calendar year 1999 in the metal and nonmetal mining industries. As of this date in 1998, there were 28 fatalities reported in these industries. This is the 6th fatality classified as Machinery in 1999. There were six Machinery fatalities in the same period in 1998.
