METAL/NONMETAL MINE FATALITY - On June 14, 2003, a 25-year repair crew foreman, with 4� years experience, was fatally injured at a sand and gravel operation. The victim was positioning a wedge bar to secure the upper liner plate in a stationary jaw crusher. Apparently the liner shifted and fell, striking the victim.

  • Identify hazards associated with the task to be performed and implement measures to ensure persons are properly protected.
  • Before removing or installing components, ensure that procedures have been implemented to secure the component against hazardous motion.
  • Require all personnel to be positioned to prevent them from being exposed to any hazards.

    This is the 12th fatality reported in calendar year 2003 in the metal and nonmetal mining industries. As of this date in 2002, there were 19 fatalities reported in these industries. This is the fifth fatality classified as Machinery in 2003. There were seven Machinery fatalities in the same period in 2002.
