METAL/NONMETAL MINE FATALITY - On October 18, 2005, a 36-year old miner with 3 years and 8 months mining experience was fatally injured at an underground potash mine. The victim had parked his articulated haulage unit and exited the operator's cab. He was found pinned beneath the haulage unit.

Stop, Look, Analyze, and Manage (SLAM) each task to identify all potential hazards. Set the parking brake before exiting mobile equipment. Do not attempt to stop moving equipment after you have exited the vehicle. When parking mobile equipment on a grade, chock the wheels or turn them into a rib.
