METAL/NONMETAL MINE FATALITY - On July 15, 2006, a 61-year-old truck driver, with 40 years experience, was fatally injured at a sand and gravel operation. The victim was in the pit area to get his first load of the shift and backed the truck into a water filled pit. The area had been flooded due to recent rains and was idle for several days making it difficult to identify the edge of the pit.

Ensure that examinations of ground conditions are conducted in pit areas before commencing work and relay potential hazards to all miners exposed. In situations/locations where travelways are not clearly distinguishable, markers or other marking techniques should be used to aid equipment operators. When conditions change, examine travelways to evaluate hazards and install barriers to limit travel of mobile equipment. Stop, Look, Analyze, and Manage, SLAM each task to identify all potential hazards. Initiate action to protect yourself when performing every task.

This is the 17th fatality reported in calendar year 2006 in the metal and nonmetal mining industries. As of this date in 2005, there were 17 fatalities reported in these industries. This is the 6th Powered Haulage fatality in 2006. There were six Powered Haulage fatalities in the same period in 2005.
