District 5 - Coal Mine Safety and Health
A Serious Non-fatal Accident
On Wednesday, October 8th, 2003, at approximately 8:10 p. m. the clean coal loadout bin at a preparation plant experienced a structural failure and collapsed. The weld between the cone and vertical section of the bin failed, allowing the cone to fall about 22 feet, collapsing the support structure below the bin, damaging the control room, and crushing the rail car under the loadout. The bin was loaded with approximately 300 tons of coal at the time of the accident; its capacity was 500 tons. Two employees located in the control room received minor injuries.
- Bins should be inspected routinely for signs of structural fatigue.
