Hoisting Accident - Brake Car Sling Broke

On September 28, 2002, a hoisting accident occurred at an underground mine in District 8. Four employees were riding in a brake car with a shearer transport-trailer attached to the inby end. As the shearer transport-trailer and brake-car were being pulled up the slope, the transport-trailer's floating axle hung on the concrete floor. The 1 1/2"-sling that connected the transport-trailer to the brake car broke. This caused the brake car to recoil up the slope and jolt the four employees in the car. This action resulted in a lost time injury to one of the employees.

Best Practices
  • Supplies or tools, except small hand tools or instruments, shall not be transported with men.
  • Use proper connecting devices when pulling material with slope car
  • Safety chains or slings must be properly rated for the load pulled
  • All safety devices on brake cars should be examined, tested daily, and maintained in proper working order
