METAL/NONMETAL MINE FATALITY - On January 5, 1995, a shovel operator (contractor
employee) with 13 years of mining experience was killed at a gold
pit. Approximately 50,000 tons of material sloughed off the
highwall of the open pit engulfing an 85-ton haul truck and a pit
shovel. The shovel operator was dead when recovered. The truck
operator was recovered with minor injuries. This is the first
fatality reported in calendar 1995 in the metal and nonmetal
mining industries. As of this date in 1994, there was also one
fatality reported in these industries. This fatality is
classified as fall of face/highwall.
