METAL/NONMETAL MINE FATALITY - On February 10, 1995, a form carpenter (contractor employee) was killed at a
limestone mill. Several employees were preparing to
install a new kiln. They were setting up forms to pour
cement when the forms collapsed on the victim. The victim
was dead when he was recovered from under the forms and
rebar one and one half hours after the accident. This is
the seventh fatality reported in calendar year 1995 in the
metal and nonmetal mining industries. As of this date in
1994, there were six fatalities reported in these
industries. This death is the second fatality classified
as falling/sliding material this year. There was one
falling/sliding material fatality during the same period
in 1994.
MSHA is prohibited by congressionally imposed budget
restrictions from enforcing the training requirements of
30 CFR Part 48 at this operation.
