METAL/NONMETAL MINE FATALITY - On October 12,1999, a 26-year-old laborer (contractor operator) with 5 weeks mining experience was fatally injured at a limestone/cement mine. The victim fell approximately 100 feet to his death from scaffolding that came apart inside a clinker silo. The victim was not tied off.
Best Practices
Scaffolding should be checked for integrity prior to use
Railings should be installed to prevent persons from falling
Persons must be tied off when working on elevated work areas whenever there is a hazard of falling
This is the 37th fatality reported in calendar year 1999 in the metal and nonmetal mining industries. As of this date in 1998, there were 45 fatalities reported in these industries. This is the 4th fatality classified as Slip/Fall of Person in 1999. There were 7 Slip/Fall of Person fatalities in the same period in 1998.
