METAL/NONMETAL MINE FATALITY -On November 11, 1999, a 49-year-old haul truck operator with 28 years mining experience was fatally injured at a crushed stone operation when he was run over by the rear wheels of his truck. The truck was on a slight grade and the victim had not set its park brake before leaving the vehicle. The truck's engine was running and the transmission was in neutral. Although the vehicle was equipped with chock blocks, they were not used. (Photo taken during MSHA investigation).

This is the 47th fatality reported in calendar year 1999 in the metal and nonmetal mining industries. As of this date in 1998, there were 48 fatalities reported in these industries. This is the 17th fatality classified as Powered Haulage in 1999. There were 20 Powered Haulage fatalities in the same period in 1998.Mobile equipment should not be left unattended unless the controls are placed in the park position and the parking brake, if provided is used.
When parked on a grade, the wheels or tracks of mobile equipment shall be either chocked or turned into a bank or rib.
