METAL/NONMETAL MINE FATALITY - On April 4, 2000, a 58 year-old lab technician with 5 years mining experience was fatally injured at a cement plant. The victim was attempting to gather a sample when he lost his balance and fell into a pit where hot precipitator dust was being dumped .

Best Practices

  • Safe access should be provided and maintained to all areas were persons work or travel.

  • Openings near travelways should be protected by railings or barriers.

  • Hazards which are not obvious to persons should be barricaded or warning signs posted.

This is the second fatality classified as Slip/Fall of Person in 2000. There was one Slip/Fall ofPerson fatality in the same period in 1999.
