Figure 8 - This image shows the head end of washer flight conveyor. As shown in this picture, this structure was not guarded at the time of the accident.

Figure 9 - This image shows the return flights above the magnetite bath while the washer is in operation. The sprocket located near the tail end of the washer is visible below the chain. These exposed, unguarded, moving machine parts were less than one foot below the victim's feet at the time of the accident.

Figure 10 - This image shows one of the attachment plates where a flight bolts to the conveyor chain. This appears to be the most likely structure to have caught the lanyard loop. A linear indentation across one side of the secondary lanyard matches the shape of this structure.
