METAL/NONMETAL MINE FATALITY - On September 28, 2005, a 39-year-old repairman, with one year and 34 weeks mining experience, was fatally injured at an underground stone mine. The victim, operating an over-the road truck that had been converted to a water truck, lost control of the truck as it descended the haulage ramp into the mine. The truck struck a stack of crib blocks and went over a berm.

Stop, Look, Analyze, and Manage (SLAM) each task. Ensure adequate pre-operational checks are conducted on all self- propelled mobile equipment and that any defects found are promptly corrected. Ensure that service brakes will stop and hold mobile equipment prior to operating mobile equipment. Exercise caution when approaching grades and operate mobile equipment in the appropriate gear at speeds consistent with the grade being traveled.

This is the 27th fatality reported in calendar year 2005 in the metal and nonmetal mining industries. As of this date in 2004, there were 20 fatalities reported in these industries. This is the 13th fatality classified as Powered Haulage in 2005. There were six Powered Haulage fatalities in the same period in 2004.
