METAL/NONMETAL MINE FATALITY - On October 22, 2007, a 22 year-old car man with 1� years experience was injured at a granite operation. He was walking across the rail yard to uncouple several loaded rail cars when he was struck by a yard locomotive. The victim was hospitalized and died on October 24, 2007.

Best Practices
  • Review procedures to ensure all possible hazards have been identified and appropriate controls are in place to protect persons. Stop, Look, Analyze, and Manage (SLAM) each task.
  • Stay in a safe location where you are not exposed to hazards when working near moving locomotives.
  • Sound a warning when approaching persons.
  • Use radios to communicate when visual contact can not be maintained.
